1、 It's best to bring a marriage certificate
Pregnancy registration is usually done before 12 weeks of pregnancy. The purpose of registration is to urge pregnant women to undergo regular check ups, facilitate doctors to record pregnancy data, provide reasonable and effective advice for pregnant women, and achieve the goal of eugenics and good fertility.
Different hospitals and regions have different requirements for the documents and materials required for card application. Some hospitals and regions do not require a marriage certificate to apply for card application, so it is best to consult in advance.
2、 Card creation process
1. Registration
Some places require advance appointment registration for filing. It is recommended to consult the relevant information of the filing location in advance and prepare accordingly.
After arriving at the filing location, queue up to get the pre inspection number. If there are family members accompanying you to file, it is recommended to have them queue up at the registration counter first.
2. Waiting for consultation
After hanging up, there will be specialized personnel to provide you with a form, and fill in your basic information according to the prompts on the form. Then sit at the entrance of the obstetrics initial consultation clinic corresponding to the registration and wait.
3. Medical consultation
The doctor will inquire about the relevant information one by one according to the content on the form you previously filled out. For example, if you are 14 weeks pregnant, the doctor will seek advice on whether you need to have a Down syndrome test, such as age, whether you have had your first pregnancy, whether you have any menstrual history, and so on.
4. Check
First, go to the payment office to pay for all the registered examination items, and then proceed with the pregnancy tests in order.
5. Follow up consultation
After completing the above four steps, submit the examination report to the doctor for recording and make an appointment for the next examination.
